Athlete Paperwork
Student athletes in ALL sports must have a physical examination along with all forms requested in the Healthy Athlete portal prior to any athletic participation. Athletic participation includes any official team practices, voluntary workouts with coaches or other athletes, any team-structured activities, and any use of the university’s weight room and fitness center. The due date for fall student-athletes is August 1st, 2026, and for semester transfer student-athletes is January 27th, 2027. In the event that you do not complete ALL forms by the specified date, there will be a $25.00 late processing fee on your student account. Please call if you have any questions.

You will receive an email from Lynsey Payne that says “Join Baker University” in the subject line. This allows you to set up your account. If you do not receive an email - Please check your spam folder and then reach out to your coach! If you have questions or troubles with your portal, please contact your sports athletic trainer.

PHYSICAL EXAMINATION
Physical Examination is completed yearly and must be administered and signed by one of the following: Doctor of Medicine (MD), Doctor of Osteopathy (DO), Physician Assistant (PA), or Nurse Practitioner (APRN). Any physician-ordered tests must be completed with documentation returned to the sports medicine department prior to athletic participation. Physicals are valid for one year from the date of the exam. Upon completion, the form must be uploaded to your Healthy Roster profile.
Your physical examination must be administered by a provider listed above. Other health providers will not be accepted, and a subsequent examination will be required prior to any participation.

Baker Sports Medicine will be offering physicals on Friday, Aug. 7th, from 5:00-7:00 p.m. in Mabee Gym on campus. Costs for physicals are $20 and must be paid upon arrival, cash or check only. All forms must be completed on Healthy Roster prior to arrival. Deadline to sign up is Aug. 5th, but signing up prior to Aug 1st waives the $25 late processing fee. Please wear comfortable clothes that you can move in. See sign-up times below:
PRIMARY INSURANCE IS REQUIRED FOR EVERY STUDENT-ATHLETE!
All student athletes are required to notify Baker University Sports Medicine Staff immediately should their insurance policy change or become invalid/inactive. All athletes must have proof of primary health/medical insurance effective August 1st, 2026, or prior to the start of the athletic season. This full coverage insurance must be carried throughout the schedule of athletic practices, contests, and participation, in and out of season. Any insurance lapse will result in the removal from participation, team roster, and the loss of continued athletic scholarship assistance.
